About Us
South West 4 is an Accountable Care Experience Unit and is specialised in general medicine with a focus on stroke and satellite telemetry management. We are one of two wards in Monash Health to have implemented the “Accountable Care Experience (ACE)” model for our patients. The ACE structure has a focus on adapting innovative change to effect and provide exceptional care to our patients. Under the ACE team structure, Registered Nurses will be encouraged to communicate in harmony with Medical, Allied Health and Pharmacy leads to implement care plans.
We encourage engagement in professional development activities including, but not limited to supporting those who would like to undertake Post Graduate Certificates with application to Medical/Surgical Transition into Specialty Practice. We support you by providing on-site education. You have access to Clinical Educators and Clinical Coaches.
About You
As an experienced RN with minimum 1-2 years of Acute Hospital experience in general medicine, you work with a patient centred approach and an excellent bedside manner. You are curious about your ability as a nurse and are willing to explore this curiosity through seeking support and mentoring from those around you. Ideally, you have current experience in stroke management, telemetry and general medicine however we certainly welcome your current experience and look forward to supporting and growing on your current clinical knowledge and exposure in an acute area.

– Current APHRA registration
– Proven Clinical experience
– A willingness to learn and grow your leadership capabilities through commitment to a portfolio
