About the Department
St. Vincent’s Mental Health (SVMH) includes an adult area mental health service for the inner city areas of Yarra and Boroondara.
The Hawthorn Continuing Care Team provides ongoing specialist treatment and support to meet the identified needs of clients who suffer with a serious mental illness and associated high level of disability in the City of Boroondara. The multidisciplinary team provides a range of clinical and therapeutic interventions using the recovery-focused Strengths Model of Recovery-Oriented Practice.
About the Role
You will provide a high standard of clinical assessment, treatment and support to clients of the St. Vincent’s integrated adult MHS and their carers, demonstrating excellence in discipline specific and recovery-focused case management skills. In addition to the clinical work a significant component of the role involves the development and maintenance of sustainable working partnerships with a range of other service providers both internal and external of the St Vincent’s’ Health organisation.
You will join a dynamic community mental health team operating under a recovery-oriented model of care, utilizing the Strengths Model of Case Management.
Duties
– Work within the recovery orientated practice using Strengths Model of Care
– Actively engage consumers in the planning, delivery and evaluation of STV services
– Provide high quality clinical services to consumers and carers
– Display active participation in professional development and ongoing learning opportunities
– Display a high standard of clinical practice and professional conduct at all times, providing clinical support to other members of the treating team and within the broader mental health service
Qualifications, Skills and Experience
– Bachelor Degree in Occupational Therapy or Social Work
– Clinical Masters in Psychology and current Registration as a Psychologist under the Health Professions Registration Act 2005 (Vic)
– Division 3 or Division 1 Nurse with Psychiatric Endorsement from the Australian Health Practitioner Regulation Agency (AHPRA)
– A minimum of at least 2 years post qualification clinical experience within Mental Health Services
– Experience in the provision of assessment, discipline specific intervention and case management services to consumers with a serious mental illness and complex needs and their carers within a recovery framework
– Highly developed interpersonal and communication skills
– Post-graduate qualifications in relevant area of specialty or in the process of achieving the same are desirable
